These are services are those which are not available on the NHS and are classed as Private, therefore, a fee is payable. The fees are set with reference to recommendations made by the British Medical Association.  A list of commonly charged fees is available on the notice board in the waiting room but please feel free to telephone the surgery on 01942 807600 and speak to reception staff in the first instance if you feel the service you intend to request from the Practice may incur a fee.  Please also note that fees may vary according to the time taken to complete the report and any administration costs e.g. photocopying/postage. Any fee applicable will be requested in advance of a non-NHS service the Practice completes on your behalf.

Examples of Non-NHS Services:

• Private sick note or short certificate (inc. fitness to attend school/games etc)
• Travel Cancellation Form Sickness/Insurance Claim Form
• Full Report from Records (e.g. driving fitness/insurance claim)
• Private Medical Examination (e.g. drivers/employment)
• Fitness to Travel Certificate
• Non-NHS Vaccinations
• Sickness/Accident Reports
• Full Medical & Report
• Insurance Proposal
• Letter to cancel travel arrangements
• Medical Report/information as required by DVLA - this may be chargeable and you should check who is responsible for the payment, is it yourself or the DVLA

Copy of medical records (separate fees may be applicable for paper and electronically (computer) held notes) - please inquire via the Practice for fees involved in obtaining copies of your medical records.
