Sick Notes

All employees are able to self-certify themselves as being too sick to work for the first seven days of illness. Forms for self-certification can be obtained from your employer or downloaded from the Department for Work and Pensions (DWP) website (http://dwp.gov.uk/)

Following the first seven day period of self-certification, a Medical Statement of Incapacity for Work (fit note) is required.  In certain rare circumstances, i.e. where a patient has just been discharged from hospital after an operation or is housebound, it may be possible to arrange a sick note without you having to be seen by a doctor - if this is the case you should telephone reception staff on 01942 807600 to discuss the matter in the first instance; in all other cases, initial/first sick notes will only be issued by arranging an appointment with the doctor.
